To answer your second question:

$('<input>').attr({
    type: 'hidden',
    id: 'foo',
    name: 'bar'
}).appendTo('form');

The same as David's, but without attr()

$('<input>', {
    type: 'hidden',
    id: 'foo',
    name: 'foo',
    value: 'bar'
}).appendTo('form');
